BDS Application Form

To proceed with the form, please make sure you have the following documents and information ready:

1. Caste Certificate: If you belong to the SC/ST or OBC category, you will need to upload a copy of your caste certificate. Ensure that the certificate is valid and meets the requirements specified by the form.

2. NEET Score Certificate: You will need to upload your NEET score certificate. This document verifies your score in the National Eligibility cum Entrance Test (NEET). Make sure you have a digital copy of your NEET score certificate ready for upload.

3. 10th, 11th, and 12th Marksheet: Prepare digital copies of your 10th, 11th, and 12th-grade marksheets. These documents demonstrate your academic performance in these respective years. Ensure that the marksheets are legible and contain the required information.

4. Transaction ID of Rs. 20000 Token Amount: You will need the transaction ID for the Rs. 20000 token amount payment. This ID serves as proof of payment and may be required for verification purposes. Make sure you have the transaction ID readily available. If you don’t have it yet, please follow the instructions provided below the form to complete the payment and obtain the transaction ID.
